OFFICE AND PHONE CONSULTATIONS
GUIDELINES
Jesse Hanley, M.D. is currently a consultant and no longer a
primary caregiver. She will support you and your primary medical doctor in
finding the causes of your health issues and find the solutions. Consultations
are $350.00 per hour and are done either in person or on the phone on Tuesdays
and Thursdays between 11:00 am and 3:00 pm Pacific Standard Time. All
cancellations require 24 hours notice to avoid a $350.00 cancellation fee.

1) Dr. Hanley would like to
have any
pertinent medical records, lab test results including x-ray or sonogram reports and prescription
medical history. We recommend that you first receive your medical records so you can
make copies of them to bring with or mail prior to your appointment. If you need to fax your
medical records, we prefer that you limit it to five (5) pages. In addition, please include
a brief 1-2 page outline of your medical history.
2) For our records, we will need your
date of birth, current address, daytime telephone number and email address along
with a
current photo for our files.
3) Please allow the Office Assistant up
to 1 week
to return your messages.
4) Once your appointment is
scheduled, you are responsible for calling, or coming to the office at your
scheduled time. There are usually only 15 minutes between consultations;
so calling or arriving on time is important. We do not give reminder
calls, so please keep a record of your appointment date and time.
5) All appointments are secured with either a
Visa, MasterCard or American Express credit card. Please have your credit
card number with a valid expiration date ready when you call to schedule your
appointment. We do accept and prefer personal checks, money orders or
cashier’s checks. If you
need to cancel your appointment, please do so 24 hours prior to avoid a fee of
$350 for the time that was reserved for you, otherwise it will be charged to
your credit card.
6) Dr. Hanley does NOT accept insurance or
Medicare. After your consultation, you will be given a superbill along with a
receipt of payment for you to submit to your insurance company for
reimbursement. If you are having a phone consultation, we will mail you the
superbill along with your credit card receipt.
